    Fosfomycin calcium is the calcium salt of fosfomycin, a broad-spectrum bactericidal antibiotic. It is a structural analog of phosphoenolpyruvate and acts by irreversibly inhibiting the enzyme MurA (UDP-N-acetylglucosamine enolpyruvyl transferase), which is essential for the first committed step in bacterial cell wall synthesis. This unique mechanism of action makes it effective against a range of Gram-positive and Gram-negative bacteria, including some multidrug-resistant strains. Primarily, fosfomycin calcium is used as an active pharmaceutical ingredient (API) in oral formulations for the treatment of uncomplicated urinary tract infections (UTIs), particularly those caused by Escherichia coli and Enterococcus faecalis. Its high urinary concentration and good safety profile make it a valuable therapeutic option. Beyond its role as an API, calcium fosfomycin can also function as a source of calcium in certain pharmaceutical preparations, falling under the category of excipients with specific mineral-providing functions.
